• We will gather your information, check your insurance benefits, answer questions you may have, and email you forms to complete.
• Complete the intake forms by logging into the patient portal
• Our office staff will contact you to set up your first Telemedicine appointment

• Complete the Contact Form by providing your name, contact information, and a brief description of why you’re seeking therapy.
• Our office will contact you to gather information, check insurance, answer any questions you may have.
